SF-GetCare allows hospital discharge planners to complete integrated intakes online.

To get started, create your user name, request a password, and set your security question. Then submit the form below to DAAS Intake staff. Once your profile is complete, you'll be able to submit completed intakes for the Transitional Care Program (TCP), long-term Home Delivered Meals (HDM), Case Management (CM), and In-Home Supportive Services (IHSS).
